The Intestinal fatty acids induce the secretion of cholecystokinin (CCK), hormone which stimulates gallbladder contraction. Orlistat (Xenical) produces a decrease on postprandial concentrations of the hormone when reducing the number of fatty acids located in the intestinal lumen. It has not been determined whether this effect could cause the formation of gallstones; this is why Orlistat (Xenical) is contraindicated in patients with cholestasis.
Orlistat (Xenical) is contraindicated in patients with chronic malabsorption syndrome due to its effects on the absorption of nutrients and vitamins.
It is contraindicated in patients with a history of nephrolithiasis by calcium oxalate with hyperoxaluria given to increasing levels of oxalate in the urine.
Because of a lack of studies during pregnancy or lactation, Orlistat (Xenical) should not be administered to these women as the outcome they might have is not known yet.
Patients with anorexia or bulimia should not take Orlistat (Xenical), since there is a risk of abuse by these patients.
It is very important to determine the causes of obesity; Orlistat (Xenical) is contraindicated in cases of obesity by hyperthyroidism. Please remember that Orlistat (Xenical) works losing 30% of fat ingested, but it has no effect on obesity caused by hormonal problems or obesity as a result of the daily intake.
