Cardiovascular diseases and Xenical

According to a Study made at the “European Congress on Obesity” (ECO) in Budapest, Xenical not only reduces considerable weight on obese persons, it also improves the cardiovascular risk factors, such as arterial pressure, glycemic control and lipid profile on patients suffering from type II diabetes and hypertension.

- Blood glucose levels
- Insulin
- Glucose dissolved
- Total Cholesterol

Patients treated with Xenical doubled their weight loss compared to the placebo treated patients, which was also notable in the waist circumference reduction.

On patients with normal results to the treatment (weight loss of 5 %- up to three months) Xenical produced arterial pressure changes, insulin, dissolved glucose, and weight opposite to the placebo.

Arterial pressure normalization on patients treated with Xenical was more than the double than the placebo treated patients.

Eating Healthy

Several population studies have established reference intakes for Energy and Nutrients for each kind of population or individual. In healthy individuals the recommended food plan is 30 Kcal per Kilogram (of Healthy Weight). It’s recommended lower If overweight (25 to 20 Kcal / Kg), each case is different. Then the total calories per weight are distributed in percentages for each nutrient.

There are also tables that divide foods into groups of milk, vegetables, fruits, cereals, meat, fats, and sugar. Each group has calculated an average input of nutrients (protein, fats and CHO). Daily intakes are calculated based on this list and then divided into three main meals and snacks.

Can I Take Xenical? - Some Tips

Overweight people (see body mass index calculation) may take Xenical (Orlistat) in addition to a well balanced food plan followed by daily exercise to reduce body fat and develop muscle mass.

Safety and efficacy for children and adolescents under 18 years old has not been established.

It is very important to have a moderately hypocaloric food plan (1200-1500 Calories), with a 15% macronutrients distribution of protein, 55% carbohydrates and 30% fat to obtain good results on your Diet Plan for Weight Loss.

To achieve good results with Xenical (Orlistat) you must follow a 6 months treatment minimum. The first month you will already start losing weight and then notice a reduction of your waist. Calculating the Body Mass Index (BMI)

Body mass index calculation (BMI) is one of the easiest ways to learn if you are within the ideal weight or not.

The formula for METERS and KILOGRAMS: WEIGHT (kilograms) between HEIGHTS (meters), a result between HEIGHTS (meters) again.

Example: 80kg / 1.75m / 1.75m = 26.12

The formula for POUNDS and INCH: WEIGHT (pounds) multiplied by 703 between HEIGHTS (inches), resulting from HEIGHTS (inches) again.

Example: 270lb * 703 / 68plg / 68plg = 41.0 BMI

Values:
Between the 18.5 Kg/m2: LOW WEIGHT.
Between 18.5 and 24.9 Kg/m2: IDEAL WEIGHT.
Between 25 and 29.9 Kg/m2: OVERWEIGHT.
Between 30 and 39.9 Kg/m2: OBESITY.
- More than 40 Kg/m2: MORBID OBESITY.

The body mass index is not always an accurate way to determine whether or not a person needs to lose weight. Here are a few exceptions:

- Body builders: because muscle weighs more than fat, people who are unusually muscular may have a high body mass index.

- Elderly: in old age, it is often better to have an index between 25
and 27 instead of an index below 25.
If a person, for example, is more than 65 years, a slightly higher body mass index may help protect against osteoporosis.

- Children: although a large number of children are obese, one should not use this index calculation to assess a child.

Available drugs for the treatment of obesity

The mechanism of action is the way in which drugs for the treatment of obesity (FDA approved) are qualified.
So are the anorexigens (decreasing appetite), termogenics (increasing energy), and inhibitors (reducing fats ingested, at a gastrointestinal level)

Anorexigens:

- Sibutramine (Reductil, Meridia, Florinef, etc.) belongs to this group. Its central mechanism of action acts by inhibiting the receiving serotonin and norepinephrine, and by increasing the sense of satiety (being full). It also increases the thermogenic activity (energy spent) through a sympathomimetic effect.  

Absorption Inhibitors:

- Orlistat (Xenical, Alli, etc.) belongs to this group. It reduces the absorption of fat up to a 30% by intestinal inhibition of lipase. Orlistat is not absorbed into the bloodstream, making it one of the safest (if not the most) drugs, with fewer side effects, and without any major problems for interaction with other medicines.  

There also are dietary fiber (glucomanano, methylcellulose, etc).
Used to produce a feeling of satiety, thus decreasing the appetite.

Dosage and Administration of Xenical (Orlistat)

Xenical (Orlistat, Roche) is available in capsules of 120 mg following administration of 1 capsule with meals 3 times daily.

The dose depends on the patient’s eating habits, the administration of Xenical could be different for each patient.

Since Xenical (Orlistat) works reducing the absorption of fats in each meal, it makes no sense to take Xenical together with meals that contain no fats (for example: rice and carrots, oil-free).

In most of South American countries the intake of fats occurs only at lunch and dinner, unlike the United States where breakfast contains a significant amount of fats consumed daily, which is why you have to select each dose of Xenical properly, always advised by your family doctor.

Xenical (Orlistat) can be perfectly skipped in foods that do not need the absorption of fats, and then taken at the next meal.

The safety of Orlistat for long-term treatments has not been demonstrated by studies (more than 2 years).

Side Effects of Orlistat (Xenical)

The Intestinal fatty acids induce the secretion of cholecystokinin (CCK), hormone which stimulates gallbladder contraction. Orlistat (Xenical) produces a decrease on postprandial concentrations of the hormone when reducing the number of fatty acids located in the intestinal lumen. It has not been determined whether this effect could cause the formation of gallstones; this is why Orlistat (Xenical) is contraindicated in patients with cholestasis.

Orlistat (Xenical) is contraindicated in patients with chronic malabsorption syndrome due to its effects on the absorption of nutrients and vitamins.

It is contraindicated in patients with a history of nephrolithiasis by calcium oxalate with hyperoxaluria given to increasing levels of oxalate in the urine.

Because of a lack of studies during pregnancy or lactation, Orlistat (Xenical) should not be administered to these women as the outcome they might have is not known yet.

Patients with anorexia or bulimia should not take Orlistat (Xenical), since there is a risk of abuse by these patients.

It is very important to determine the causes of obesity; Orlistat (Xenical) is contraindicated in cases of obesity by hyperthyroidism. Please remember that Orlistat (Xenical) works losing 30% of fat ingested, but it has no effect on obesity caused by hormonal problems or obesity as a result of the daily intake.
